Travel Health Questionnaire
To help us offer the appropriate advice, please fill out the online form before coming to see the nurse. Then hand in the completed form to reception. You will then be told when to contact us back to make an appointment to see the nurse for any vaccinations you will need. The nurse will need to check the completed form before an appointment is made.
Please contact the surgery in advance as the vaccines should be given at lease 6 weeks before travel.

Thomas Linacre Centre - the Blood Taking Department is open from 7.30am – 4.45pm Monday to Friday. Please be aware that all blood tests must be completed by 4.45pm. On days when large numbers attend, patients waiting after 4.00pm may be asked to return the following working day. Please also note that we do not take blood from children under the age of 12. Appointments for children under the age of 12 are available by ringing Platt Bridge Paediatric Clinic on 01942 481345 or 01942 481151.
Leigh Walk in Centre -
From the 1st of June 2017 appointments for the phlebotomy clinic which runs 7am to 9am at Leigh Walk In Centre must now be booked via 01942 483483 and NOT via the Walk in Centre reception.
